Telehealth Consent

By using HealOvi telehealth services, you consent to receive healthcare services through telehealth. Telehealth involves the use of electronic communications, digital forms, video, audio, messaging, images, remote monitoring, and other technology to allow healthcare providers and patients to interact when they are not in the same physical location.
1. Nature of Telehealth
Telehealth may include online intake questionnaires, secure messaging, video or audio visits, photo or document uploads, review of medical history, review of medications and allergies, remote treatment recommendations, prescription decisions when clinically appropriate, lab ordering or review, care coordination, and follow-up communication.
2. Potential Benefits
Telehealth may offer convenient access to care, faster communication with providers, reduced travel time, more flexible scheduling, ongoing follow-up, and access to certain services from home.
3. Potential Risks
Telehealth also has risks. A provider may not have access to your full medical history. A physical exam may be limited or unavailable. Technology may fail or delay communication. Images, forms, or messages may be incomplete or unclear. A condition may be misdiagnosed or require in-person evaluation. Treatment may be delayed if you do not respond to messages. Privacy or security risks may exist despite safeguards. Prescriptions may not be appropriate without additional information. A provider may determine that telehealth is not appropriate for your condition.
4. No Emergency Care
HealOvi is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
5. Your Responsibilities
You agree to provide truthful, accurate, complete, and current information; update your provider about new symptoms, medications, allergies, pregnancy, medical conditions, side effects, or changes in health; respond to provider messages and requests for information; follow treatment instructions; read medication labels and patient information; seek urgent or emergency care when needed; use the Services only for yourself unless expressly authorized; and be physically located in a state where the provider is licensed and the service is available.
6. Provider Discretion
A provider may decline to treat you through telehealth, request additional information, require labs, recommend in-person care, refer you to another provider, or discontinue treatment if telehealth is not appropriate.
7. Prescription and Pharmacy
Some HealOvi services may include access to prescription medications when clinically appropriate. A prescription is not guaranteed. A licensed healthcare provider will determine whether a medication is appropriate based on your medical history, symptoms, current medications, allergies, risk factors, applicable guidelines, and other relevant information. Prescriptions may be sent to a HealOvi-affiliated pharmacy partner, pharmacy network, or a pharmacy selected by you where available. Refills are subject to clinical review, pharmacy availability, legal requirements, payment status, and provider discretion.
8. Compounded Medications
Some HealOvi services may involve compounded medications when prescribed by a licensed healthcare provider and dispensed by a licensed pharmacy. Compounded medications are not FDA-approved. The FDA does not review compounded medications for safety, effectiveness, or quality before they are marketed. Compounded medications may carry risks, including variation in potency, quality, or consistency; contamination risk; different inactive ingredients; limited clinical evidence for certain formulations; differences from FDA-approved medications; and unanticipated side effects. A compounded medication is not the same as an FDA-approved brand-name medication and should not be described as FDA-approved, generic, equivalent, or identical. By using a service that involves compounded medication, you acknowledge that you understand the medication may not be FDA-approved and that you have had an opportunity to ask questions.
9. Privacy
Your information will be collected, used, shared, and protected as described in the Privacy Policy, HIPAA Notice of Privacy Practices where applicable, and other applicable privacy notices.
10. Withdrawing Consent
You may withdraw your consent to telehealth by contacting us via the contact form at healovi.com/contact. Withdrawal of consent may limit your ability to use HealOvi telehealth services.
11. Acknowledgment
By using HealOvi telehealth services, you acknowledge that you understand the benefits, risks, alternatives, and limitations of telehealth and consent to receive care through telehealth.
